Co-pay Assistance Terms and Conditions

  • The patient must have a valid prescription.
  • The program is not valid for prescriptions that are eligible for reimbursement, in whole or in part by Medicaid, Medicare, or other federal or state healthcare programs.
  • This program is not valid for prescriptions that are eligible for reimbursement by private insurance plans or other health or pharmacy benefit programs that reimburse you for the entire cost of your prescription drugs.
  • Program covers the cost of product only.
  • Each patient is limited to one active offer at a time and the offer is not transferable.
  • The program cannot be combined with any other rebate, coupon, free trial, discount or similar offer for the specified prescription.
  • This program is not health insurance.
  • This offer is good only at participating pharmacies in United States and its territories as allowed by law.
  • Azurity reserves the right to rescind, revoke, or amend the program without notice.

Calendar and Clock Bridge Program

If insurance approval takes more than 48 hours, the Bridge Program may help cover the gap.

  • Covers the gap when insurance approval takes more than 48 hours
  • Up to 30 days of medication at no cost
  • Call to see if you qualify

Bridge Terms and Conditions

  • The patient must have a valid prescription.
  • Valid only for patient’s first prescription and when insurance approval has been delayed. Existing and restart patients are not eligible.
  • Prescriber must complete and submit Bridge enrollment form to enroll patient in the program.
  • Patient or provider cannot submit the value of the free product as a claim for payment to any third-party payer.
  • Patient or provider must contact program if insurance changes or coverage becomes available, at any point during the program.
  • Program covers the cost of product only.
  • The program is valid only in the United States and its territories as allowed by law.

Piggybank Patient Assistance Program

Your access to EPRONTIA® shouldn’t be limited by your ability to pay. The Azurity Solutions Patient Assistance Program helps eligible patients get the financial support they need.

Patient Assistance Program (PAP) Terms and Conditions

  • The patient must have a valid prescription.
  • Prescriber must complete and submit a PAP enrollment form for every patient.
  • Patients whose health insurance plan or employer requires them to go through a third-party Alternative Funding Program (AFP) and apply to the PAP as a condition of, requirement for, or prerequisite to coverage of product will not be eligible for assistance from this program.
  • Patient must meet the eligibility and income requirements to qualify for the program.
  • The program cannot be combined with any other rebate, coupon, free trial, discount, or similar offer for the specified prescription.
  • The program is valid only in the United States and its territories as allowed by law.
  • Azurity reserves the right to rescind, revoke, or amend the program without notice.
